(EMR), such as Patient Demographics: Patient Race, Ethnicity, Social History , Medication : RxNorm Codes, NDC Codes, Diagnosis codes (ICD 9/10), SNOMEDCT, CPT codes, HCPCS, Vital Signs, Plan of Care : Cessation and counselling data, Lab Results and Lab Test Data, LOINC, Patients Notes, and Insurance related data.
